You may obtain a copy of your medical records for routine medical follow-up by completing a release of information form and submitting a $25.00 check payable to

Patrick J. McNamara, MD PA. Your medical records will be forwarded to you within 2 weeks of receipt of your medical release form and check. The fee is to cover the cost of copying and postage.
